

To be considered for admission to "My Sister's Place", the following steps must be completed:

A "Referral Request Form" for acceptance in residency program must be completed by the requesting agency, and submitted for consideration, review and acceptance. A referring agency or department recommending the candidate for "My Sister's Place" must complete this form. This form will also permit us to assess the candidate's needs.

A "Resident Intake Form" must be completed before acceptance can be granted into the home. This form will supply the home's administration with personal and professional information.

Medical & Prescription History
Upon acceptance into the program, a "Letter of Agreement" must be completed and signed. The "Letter of Agreement" will define the responsibilities of both the candidate and "My Sister's Place". The following areas will be addressed:

Nature and purpose of the agreement

Responsibilities of "My Sister's Place"

Responsibilities of the residents

Termination of this program and its Letter of Agreement
A "Resident Intake Checklist" will be completed for each resident upon her admittance into "My Sister's Place"

All candidates must produce any medical insurance they have. Candidates with medical insurance will be required to follow their medical plan. Candidates without medical insurance will be referred.

All candidates will be subject to random drug testing (at the discretion of the House Monitor).

All medication must be reported to House Monitor.
